Editor's note

'Expedited' means overnight return when a pre-addressed, pre-paid airbill is included with a mailed request; no online submission is offered. Page wording for in-person payment is 'credit, debit, check or order money' (apparently money order). Mailed requests must include a self-addressed stamped envelope.
